Agencies may forward Production PIV Cards populated with data to be evaluated for compliance with FIPS 201 requirements by completing the following steps: 1) Create a production PIV Card. 2) Download and complete GSA CARD DATA VALIDATION FOR AGENCIES 3) Scan and email completed form (step 3) to fips201.epmanager@gsa.gov 4) Send card via mail to: Mailing Address (Send Card here): GSA 1800 F Street N.W. room 2239 Washington, DC 20405 (P) 202.501.1123 The Evaluation Program will send you an email confirmation, when your card is received. Your test results will be forwarded via email to the government representative listed on the form. Card will be returned via mail to government representative within 5 days of test completion. |
